Social Determinants of Health (SDOH) Assessment

User Research: Designed the SDOH assessment that surfaces social barriers impacting health and guides community health workers to personalized resource referrals.

Developed a SDOH visual cue displayed at each medical station to streamline team awareness and encourage timely referrals to the community health worker.
